This page provides forms and procedures for many Graduate School functions—including, but not limited to dissertation submission and approval forms; faculty and staff forms related to graduate student defenses, application for candidacy, and results of oral defense; and adds, drops, petitions, requests for leave, and more.
Advisor/Advisee Agreement
We require all thesis students and their advisors to read and sign an agreement about the advisor/advisee relationship, expectations, and requirements. This is not a mandated mentoring rubric, but prompts a discussion between advisors and advisees prior to thesis work. You can read the Advisee Agreement here.
The Graduate School Policy on Professional Conduct
In keeping with Baylor University’s commitment to mutual respect and personal integrity, the Graduate School expects that all students will conduct themselves in a manner befitting their professional identity.
Dismissal Appeal Form
Students wishing to appeal a dismissal from their graduate program may utilize this form.
